Terms of Use
Last updated: November 4, 2026
These Terms of Use (“Terms”) are an agreement between you and Dr. Kareem F. Samhouri Fitness LLC, which provides Internet Physical Therapy (“we,” “us” or “our”). They apply when you use www.internetphysicaltherapy.com (the “site”) or any of our services. By using the site or our services, you agree to these Terms, our Privacy Policy and our Payments & Cancellations policy. If you don’t agree, please don’t use the site or our services.
1. Not for emergencies
If you may be having a medical emergency, call 911 or go to the nearest emergency room. Our services, website, messages and patient portal are not monitored around the clock and are not for urgent or emergency care.
2. Our services
- Physical therapy. Your Physical Therapist (currently Dr. Kareem Samhouri, PT, DPT) provides 1-on-1 physical therapy by live video to patients who are physically located in a state where your Physical Therapist is licensed at the time of each visit, currently Arizona and Pennsylvania. Physical therapy starts with an evaluation. A therapist-patient relationship begins when your evaluation starts. At the end of it, your Physical Therapist will tell you whether ongoing care by video is right for you. Before your first visit you will be asked to agree to our Telehealth Informed Consent.
- Coaching and training. Personal training, nutrition coaching, and health and epigenetics coaching are not physical therapy. They do not evaluate, diagnose or treat pain, injuries or medical conditions, and nutrition coaching is general guidance, not medical nutrition therapy.
- Educational programs. Live workshops, courses, memberships, videos and other content we offer teach general principles to a group. They are not individualized care and do not create a therapist-patient relationship. See our Disclaimer.
We may decline to provide, or stop providing, physical therapy by video if a Physical Therapist decides it isn’t safe or appropriate for you, in which case we will tell you and recommend in-person care.
3. Who can use our services
You must be 18 or older to use the site and our services. You agree to give us accurate information, including the state where you will be during each visit, and to tell us promptly if anything changes. People with Medicare should read the Medicare section of our Payments & Cancellations policy before booking.
4. Booking, accounts and other services we use
Appointments, payments, intake forms and video visits take place on Jane, our patient portal, and programs may use a separate checkout or course platform. Those services have their own terms, which also apply when you use them. You are responsible for keeping your login details private and for activity under your account. Please don’t share course or membership logins, recordings or materials with others.
5. Payments, cancellations and refunds
Prices, payment timing, cancellations, refunds and your right to a Good Faith Estimate are explained in our Payments & Cancellations policy, which is part of these Terms. We do not bill insurance.
6. Your safety during sessions
Exercise carries some risk of injury. During video visits and live sessions, use a clear, safe space, wear suitable clothing and footwear, stop and tell us if anything causes pain, dizziness or shortness of breath, and follow the safety guidance your Physical Therapist or coach gives you. If you have a medical condition, talk to your doctor before starting an exercise program.
7. Emails and texts
By giving us your email address or mobile number, you agree that we (and Jane, for appointment reminders) may contact you about your appointments, purchases and account. We send marketing emails only with your consent, and we don’t send marketing texts. Reply STOP to stop texts, or tell us any other way; use the unsubscribe link to stop marketing emails. Message and data rates may apply. See our Privacy Policy.
8. Using the site
You agree not to misuse the site: for example, not to break the law with it, interfere with its security or operation, try to access accounts or data that aren’t yours, copy it with automated tools, or upload anything harmful.
9. Our content
The site and everything we create for our services, including text, videos, exercise programs, workshop and course materials, and our name and logo, belong to us or our licensors and are protected by law. We give you a personal, non-transferable permission to use them for your own non-commercial use. You may not copy, share, sell or publish them without our written permission.
10. Testimonials
We share patient stories only with your written authorization. If you send us feedback or ideas, we may use them without owing you anything, but we will never identify you without your permission.
11. Disclaimers
The site and its content are provided “as is” for general information. We work to keep them accurate and available, but we don’t promise that they will always be complete, current or uninterrupted. Results from physical therapy, coaching and programs vary from person to person, and we cannot guarantee any particular outcome.
12. Limitation of liability
This section does not apply to claims about the physical therapy care you receive from us. For the site, our educational content, coaching and programs, to the fullest extent the law allows, we are not liable for indirect, incidental, special or consequential damages, and our total liability for any claim is limited to the amount you paid us for the service involved in the 12 months before the claim. Nothing in these Terms limits any liability that cannot legally be limited.
13. Your responsibility to us
If you misuse the site or break these Terms, you agree to cover reasonable losses and costs this causes us, to the extent the law allows.
14. Disputes
If you have a concern, please contact us first; most problems can be solved quickly. These Terms are governed by the laws of the State of Arizona, without regard to its conflict-of-law rules. Any dispute that we can’t resolve together will be decided by the state or federal courts located in Arizona, and you and we agree to those courts’ jurisdiction, except that a claim about physical therapy care may also be brought in the state where you received that care. Where the law of the state where you received care gives you rights that can’t be waived, those rights still apply.
15. Changes and ending these Terms
We may update these Terms from time to time. We will post the new version here with a new “Last updated” date. If a change is significant, we will tell you by email or with a notice on the site before it takes effect. Changes never affect a service you have already paid for. We may suspend or end your access to the site or our services if you break these Terms. If any part of these Terms can’t be enforced, the rest still applies. These Terms, together with the policies they mention, are the entire agreement between you and us about the site.
